The following information has been given to us regarding our Flu Clinic to be held on October 20th, 4-7PM! Family members must register online if they would like to receive the vaccine.
*The online registration process for families IS NOT TO BE USED TO REGISTER STUDENTS ENROLLED IN THE SCHOOL. Students will be sent home with paperwork to be completed and sent back to school.
No online consent forms will be accepted for students enrolled in your school. Online registration is only used to register family members not enrolled in the school.
